How to create an on-line questionnaire

Types of questions

Using pre-defined HTML form types, it is possible to create interesting questionnaires. The following question types have been defined and are available for use in i-structures:

Type 1b: drop-down list with automatic blank default

Type 1b is for drop-down lists of possibilities. In this variant of type 1, the first, blank solution is created automatically.
How many columns are there in this table?

Please answer the question

Type 3b: Radio button list

Type 3 is for radio button lists of possibilities. Only one possibility can be selected. Selection cannot be cancelled.
How many columns are there in this table?

1 column
3 columns
5 columns

Type 8b: Single line answer (numeric or textual)

Type 8b gives the user the possibility to enter an answer using the keyboard. The answer can be numeric or textual and the width can be adapted depending on the length of the expected answer. Evidently, there is no list of possibilities to enter in column 4, nor a list of comments in column 5.

Type 9b: Multi-line answer (textual)

Type 9b is for more complex answers. Obviously, such an answer will need to be corrected by a human being, and cannot be used in an auto-corrected questionnaire.
What do you think of the possibilities offered so far?
